Responsibilities
An exciting opportunity has become available within the North side of Kirklees for an experienced, motivated, and compassionate Registered Nurse to join our Community Nursing Network as a Band 6 Senior Community Nurse (37.5 hours per week, full time).
This is an excellent opportunity to take on a leadership role, supporting our teams to deliver exceptional care across North Kirklees and Greater Huddersfield.
As a Band 6 Senior Community Nurse, you will combine clinical expertise with leadership skills to ensure safe, effective, and person-centred care. You will act as a role model, mentor, and decision-maker, supporting colleagues, driving improvements in patient outcomes, and providing operational leadership in the absence of the Team Leader.
Key Responsibilities
- Lead and coordinate care for patients with complex needs, ensuring high standards of clinical practice.
- Provide guidance, supervision, and mentorship to Band 5 nurses and wider team members.
- Delegate tasks effectively to ensure efficient use of resources and timely delivery of care.
- Support service development initiatives and contribute to quality improvement projects.
- Act as a point of escalation for clinical decision-making and safeguarding concerns.
- Promote self-care and independence for patients living with long-term conditions.
- Collaborate with multidisciplinary teams to prevent unnecessary hospital admissions.
- Provide operational and clinical leadership in the absence of the Team Leader, supporting the coordination of staffing, workload, and service delivery.
- Act as a positive role model, helping to create a supportive team culture focused on quality, learning, and continuous improvement.

What We're Looking For
A Registered Nurse with substantial community nursing experience and a passion for leadership.
- Strong clinical knowledge with the ability to manage complex caseloads.
- Excellent communication, problem-solving, and decision-making skills.
- Confidence in delegation, coordination, and team leadership.
- A proactive approach to service improvement and innovation.

Working Pattern
This is a full-time position of 37.5 hours per week. Our service operates between 8am and 9pm, with core hours of 8am to 5pm. Successful candidates will be expected to work a minimum of one weekend in four and undertake two late shifts (1pm to 9pm) within a four-week period.
This role is not applicable for sponsorship, and candidates must be able to demonstrate their right to work in the UK.
