North and East Wiltshire Community Mental Health Team
The North and East Wiltshire Community Mental Health team is seeking a registered mental health practitioner to join their multidisciplinary team. The team comprises a talented pool of both registered and non-registered mental health professionals.
About the Role
We have transitioned away from the use of care programme approach (CPA) and now work with service users and carers under the “your team, your conversation, your plan" approach. This is a strength-based, intervention-led model of care, that supports the individual to address their own identified needs. We support the conversation through the use of a personal well-being and safety plan, as well as the use of patient-related outcome measures (Dialogue+).
Responsibilities
This post is within the psychotic pathway and would be working with service users who have identified associated needs that require the attention and support of secondary mental health care services.
The successful candidate would hold a caseload of service users and would become their identified keyworker. This is a fulfilling and exciting job that offers day-to-day variety.

Supervision
The team has an established supervision structure which offers individual, peer, and team supervisions. The team values these spaces to aid patient care planning and staff development.
