An exciting opportunity has arisen for a full time Band 7 Advanced Nurse Practitioner to join the Yewcroft CMHT.
Our community mental health service is a well-established team and as part of the BSOL community transformation programme, we are looking to increase our Advanced Nurse Practitioner service. This is an exciting opportunity to be at the heart of national change and developments in the delivery of community mental health care.
We are looking for an enthusiastic and dynamic individual, who is able to work within a multi-disciplinary team, working alongside other professionals and striving for the best outcomes for individuals within the local community that the team serves.
- Responsibilities:
- Provide mental health triage, assessment, and outcomes-based intervention for service users, including nurse prescribing within the CMHT.
- Identify needs and the best course of advice, support, or intervention.
- Promote psychosocial wellbeing by identifying and then supporting pathways for service users and carers.
- Ensure robust communication across BSMHFT and primary care, in accordance with each individual service user's needs, by providing support and advice to all colleagues as required, including participation within the MDTs.
