Job Summary

Are you an experienced registered nurse with a passion for delivering outstanding patient-centred care and supporting complex discharge pathways? University Hospitals Birmingham is seeking a motivated Continuing Healthcare (CHC) Assessor to join our Community Services team in Solihull.

This is an exciting opportunity to play a key role in assessing, reviewing and coordinating NHS Continuing Healthcare and Funded Nursing Care in line with the National Framework. You will work collaboratively with patients, families, community teams, acute services, social care and care providers to ensure timely, high-quality decision-making and positive patient outcomes.

As a senior autonomous practitioner, you will undertake complex assessments, support discharge planning, complete Decision Support Tools, facilitate multidisciplinary working and contribute to service development, quality improvement and staff education. The role offers the opportunity to influence pathways, improve patient experience and support delivery of integrated neighbourhood care.

We are looking for a registered nurse with substantial post-registration experience, specialist clinical knowledge, excellent communication skills and a strong understanding of NHS Continuing Healthcare. Experience of working across organisational boundaries and managing complex clinical situations is essential.

Main Duties, Tasks & Skills Required

Person specification

Qualifications

Essential

  • Registered Adult / Child Nurse on the NMC Register (Field dependent upon role)
  • Evidence of post registration CPD / Post registration qualification in (appropriate area of speciality or professional knowledge supplemented by specialist experience and short courses to FHEQ Level 6 (Degree level) or Level 7 (master's level) equivalent.
  • Recognised mentorship course

Desirable

  • To have or working towards a MSc/ master's qualification
  • To have or working towards advanced health assessment module -- where applicable to the role
  • Non -- Medical Prescriber/ willingness to undertake - where applicable to the role

Experience

Essential

  • Evidence and ability to revalidate as required by the NMC
  • Substantial years post registration experience as a Clinical Nurse Specialist at B6 or above in defined or associated speciality
  • Extensive experience of working autonomously at a specialist level
  • Evidence of achievements and career development in current post and of on-going professional development/ competence.
  • Demonstrate expert knowledge underpinned by theory and CPD relevant to speciality.
  • Experience and enthusiasm for developing educational programmes and teaching / assessing and supervising others in a clinical setting.
  • Working in a multi-disciplinary and cross agency work environment.
  • Experience of patient service user advocacy role
  • Can demonstrate expert nursing knowledge with an ability to assess, plan, implement and evaluate specialist care delivered to patients and give advice and information to patients / relatives/ care staff
  • Awareness of current relevant NHS policy context relating to the speciality
  • Has understanding / experience of the principles of Safeguarding
  • Knowledge of corporate and clinical governance

  • Ability to work a varied shift pattern where required
  • Ability to act on own initiative
  • Must have a full in date UK car driving license with access to a vehicle, successful candidate must have business car insurance
