Job Overview
The post holder will develop the knowledge and expertise within Surgery and support the wider pharmacy team to deliver a comprehensive and consistently high-quality clinical service to surgical wards, as well as being an integrated member of the parenteral nutrition (PN) multidisciplinary team and actively participating in PN ward rounds.
The post holder will work closely with the Lead Pharmacist for Critical Care to develop and implement specialist clinical pharmacy services to surgical patients within The Princess Alexandra Hospital NHS Trust.
This role also involves working closely with the Lead Surgical Pharmacy Technician to deliver a patient-focused clinical pharmacy service to patients within the elective Surgical Wards.
The role will also include weekend working and bank holidays as part of the pharmacist weekend working rota.
Main duties of the job
- To deliver, develop and evaluate clinical services to the adult elective surgical wards at The Princess Alexandra Hospital NHS Trust.
- To assist in the provision of a high-quality service to the patients and staff within the Surgical and Critical Care directorate.
- To assist the Lead Pharmacist for Critical Care with the training of rotational band 6 Pharmacists and support in the cover of pharmaceutical aspects of care in Critical Care.
- To be an active member of the Parenteral Nutrition Team and regular attendance in the daily PN ward rounds.
- To provide expert advice on pharmaceutical issues, utilising advanced specialist knowledge in surgical matters.
- Be responsible, on a day-to-day basis, for medicines reconciliation and medicines optimisation for patients on the general wards at The Princess Alexandra Hospital NHS Trust.
- To promote the safe, clinically effective and cost-effective use of medicines.
- To undertake audit/research and act as a source of pharmaceutical expertise and promote cost-effective use of medicines to other healthcare professionals.
- Ensure formulary compliance and support with formulary applications relevant to surgery.
Service Delivery Responsibilities
Detailed job description and main responsibilities
- Work closely with the Lead Surgical Pharmacy Technician to deliver a patient-focused clinical pharmacy service to patients within the elective Surgical Wards.
- To develop and evaluate clinical services to the adult elective surgical wards at The Princess Alexandra Hospital NHS Trust and implement areas for improvement to support patient flow and patient experience.
- To support the provision of a clinical pharmacy service to the Trust Adult Critical Care Unit in the absence of the Critical Care Pharmacist.
- Provide a clinical pharmacy service and support medical optimisation to Level 1 patients based on the Unit.
- Contribute to the timely discharge of patients being discharged from the surgical wards by ensuring TTAs are prepared prior to discharge.
- To improve patient experience by ensuring patients by providing patient counselling and education of medications prior to discharge.
- To work as part of the multidisciplinary team to support medical and nursing staff, as well as allied healthcare professionals.
- Creation and implementation of surgical guidelines as necessary.
- To monitor and evaluate the pharmacy services provided in accordance with the set objectives to ensure that they are of a high quality and that departmental standards are met.
- Participate in audits and service reviews in accordance with the set objectives agreed with the Lead Pharmacist for Critical Care.

Clinical Pharmacy Duties
- Provide high-quality clinical pharmacy input as part of the multidisciplinary team within the directorate to improve and enhance the quality of patient care.
- Ensure that medication histories and patients’ own drugs are checked on admission.
- Reconcile patients’ regular medication and professionally screen prescriptions, thus taking professional responsibility, as required by legislation.
- To prioritise clinical interventions in accordance with patients’ individual needs.
- To provide timely and expert clinical pharmaceutical advice to medical and nursing staff to enable them to prescribe and administer medicines for inpatients in a safe, appropriate and efficient manner.
- To help with the monitoring and implementation of clinical management guidelines such as the antimicrobial guidelines and adherence to Trust Formulary.
- To work with the Lead Pharmacist for Critical Care to develop and input into clinical prescribing guidelines, care pathways, care bundles, patient group directions, implementation of NICE and other guidance in line with local and national directives.
- To undertake counselling and provision of information on medication-related issues, delivering highly sensitive information to a vulnerable sector of the community, both patients and carers.
- To train and work as an independent prescriber (IP) according to Trust policies and within your identified scope of practice, if required.
- To monitor, audit and report on any medicines-related incidents, complaints or clinical governance issues and explore ways of minimising the risk of future re-occurrence or risk to patients and staff. Make appropriate recommendations to support improvement in these areas.
- To undertake clinical audits and to participate in undertaking research, audit or development work for service improvement.
Medicines Management & Service Development
- To carry out medicine reconciliation and medicines optimisation for patients on the general wards at PAHT.
- Assist the Lead Pharmacist Education and Training/Deputy Chief Pharmacist’s initiatives to address identified prescribing issues and promote cost-effective prescribing and drug expenditure control.
- Assist the senior pharmacist(s) in investigating issues arising from medication incidents.
- Support the development, implementation and monitoring of guidelines, protocols and standards to ensure safe and cost-effective use of medicines.
- Contribute to continuous quality improvement processes within the Assessment Unit and in the Trust through clinical governance and risk management issues.
- Contribute to any audits applicable to general wards.
- Provide information and data for Pharmacy department key performance indicators (KPIs) to demonstrate activity and quality.
- Assist with Controlled Drugs management on the general wards, including regular CD stock checks as per Trust policy, return of expired and unused patients own controlled drugs and appropriate stock levels.
- To assist with the training and assessment of pre-registration pharmacists and other pharmacy staff as required.
- To participate in education and training schemes and meetings organised within the Pharmacy and Cancer Services Departments.
- To undertake continual professional development and maintain a CPD portfolio, in line with requirements of professional body, including continuing education and attendance at appropriate courses and study days.

Managerial Duties
- To supervise pharmacy staff as appropriate to job role to ensure competencies to perform their duties.
- Clinical professional supervision of junior pharmacy staff.
Managing Self
- Participate in regular supervision.
- Attend all mandatory training.
- Participate annually in identifying, developing and agreeing your own development plan with your Line Manager using the Trust Appraisal.
- Comply with all Trust policies, procedures and protocols.
- Seek advice and support from Line Manager whenever necessary.
- Maintain professional conduct including appearance at all times.
- Ensure maintenance of Professional Registration.
General Pharmacy Duties
- To provide a high-quality, patient-focused pharmaceutical service to in-patients, and outpatients.
- To have a working knowledge of all procedures, legislation or national guidelines relating to the storage and dispensing of unlicensed medicinal products and controlled drugs.
- Dispense or check in the Pharmacy Dispensary as required.
- To input prescription information using the Pharmacy Computer system.
- To be able to use the computerised e-prescribing & medical notes system used in the Trust.
- To dispense and supply drugs to patients (including using the automated dispensing system (‘Robot’)).
- To provide evaluated pharmaceutical advice and information, to all health care professionals and patients of the Trust using on-line data bases and primary sources.
- To have a working knowledge of pharmacy stock control and ordering systems.
- To collect and handle prescription monies according to Government and Trust policy.
- To participate in all relevant departmental meetings and contribute to effective communication within the department.
- To be aware of and apply, relevant legislation such as the Health and Safety at Work Act, Control of Substances Hazardous to Health, GMP and Medicines’ Act.
- To at all times practice in accordance with the Code of Ethics of the General Pharmaceutical Council.
- To uphold the security of drugs and the premises at all times.
Other Duties
- To be actively involved in undertaking projects and audits to continually review and improve the quality of service provision.
- To undertake any other duties commensurate with the grade as requested by the chief pharmacist/deputy.
