Rotational Advanced Pharmacist
We are looking for an experienced and enthusiastic Rotational Advanced Pharmacist to join our Pharmacy team on a fixed-term basis.
This is an excellent opportunity to take a senior clinical role across Medicine, Care of Older People and the Acute Admissions Unit, providing expert pharmaceutical advice and supporting the safe and effective use of medicines.
You will work closely with multidisciplinary teams, contribute to service development and medicines optimisation, and support the development of junior pharmacy colleagues.
We are looking for a confident pharmacist with strong hospital experience, excellent communication skills and a passion for delivering high-quality patient care.
Responsibilities
- Assist the lead pharmacists in the delivery, development and evaluation of a high-quality clinical pharmacy service for Older People, Medicine and Acute Admissions Unit.
- Provide expert advice on pharmaceutical matters, undertaking risk management and ensuring compliance with medicines legislation.
- Participate in clinical audit and practice research.
- Undertake training of pharmacy staff and other members of the multi-disciplinary team.
- Actively deprescribe as a fully independent pharmacist prescriber and provide training in the use of deprescribing tools to support the reduction of polypharmacy in frail patients as per the Trust and departmental policies and procedures.
