We are seeking highly motivated, experienced and enthusiastic Advanced Clinical Practitioners to join our team to provide an enhanced and quality Out of Hours service on behalf of Harrogate and District Foundation Trust.
We would like to offer the opportunity for individuals with extended skills in primary care to apply for a ‘bank role’ providing the flexibility to apply for adhoc shifts based in the Harrogate, Ripon, Northallerton and Catterick. The service is operational weekdays 6pm – 8am and provides 24hr cover from 6pm Friday to 8am Monday and bank holidays.
The successful applicants will be responsible for ensuring that the highest standard of clinical care and initiate clinical assessment including telephone triage and consultation, diagnosis, treatment, referral using a range of advanced skills and expert knowledge for patients presenting with undifferentiated and undiagnosed conditions.
The post holder will undertake telephone triage assessments, see patients in the primary care centre and undertake home visits where clinically appropriate.

The post holder will assess, diagnose and treat or refer as appropriate, patients presenting with undifferentiated and undiagnosed conditions.
