Community Mental Health Nurse, Band 6

We are currently seeking an experienced Community Mental Health Nurse, at Band 6 to work via a Bank Contract, based in Hemel. You will be joining our Temporary Staffing Team within HPFT - Hertfordshire Partnership University NHS Foundation Trust.

About the Role

The post-holder will be responsible for the clinical and operational day to day service delivery, ensuring that an individualised comprehensive assessment, and Care Management service, is effectively delivered.

Responsibilities

  • Clinical and operational supervision of junior nursing staff and students as delegated by the team manager.
  • Supporting the Team Leader.
  • Delivering a quality service that is effective and efficient in line with the organisation’s objectives and performance indicators.
  • Contributing to strategic service development working collaboratively with other agencies.
  • Formulating a care plan and risk assessment, reviewing its effectiveness and re-plan using evidence-based interventions to best effect, in partnership with the service user.
  • Initiating, planning and delivering effective risk management systems within the clinical area.

Requirements

  • Valid Professional Registration - NMC - and the ability to evidence this.
  • Ability to undertake assessments of the need of service users who meet the service eligibility criteria, and their family.
  • Identifying and responding to acute presentations in order to manage risk and promote the safety and well-being of the service user.

The role is based in Hemel Hempstead and possibly St Albans. Travelling within the local area and across Hertfordshire when required, so you will need to be a car driver.
