Job Title: Bereavement Care Coordinator

Contract Type: Fixed Term

Fixed Term Contract End Date: December 2027

Purpose for Fixed Term: Maternity / Shared Parental / adoption Leave Backfill

Main Duties of the Job

This is an exciting opportunity to join our Bereavement and Family Support team as we continue to develop ways to meet the needs of our end of life patients and their families.

The role is pivotal to our service in capturing and sharing information sensitively and quickly to enable the deployment of time critical support in a busy and demanding department. Efficient and empathic coordination of our resources is key as we work together to meet demand, and no two days are ever the same.

The role reports to the Bereavement Lead and supports the Bereavement Specialists by sourcing, capturing and sharing key information to enable accurate and time critical decision making, along with administrative tasks.

Detailed Job Description and Main Responsibilities

The successful candidate will be responsible for:

  • Co-ordinating an efficient, accurate, compassionate and timely response to patients at the end of their lives as well as their families/friends experiencing anticipatory grief or those in the aftermath of a bereavement.
  • Responding effectively to referrals into the service in varied forms from close working relationships with other UHS teams and wards, often within a tight timescale with emotive situations, ensuring all referrals are actioned.
  • Answering telephone, face to face and email enquiries compassionately in a timely manner.
  • Compile accurate and cleansed data as well as contemporaneous notes, using Trust systems and Excel spreadsheets to produce user friendly reporting to end users.
  • Work closely and collaboratively with other teams across the Trust such as mortuary, Medical Examiners Service, PALS, Complaints etc to optimise excellent care.
  • Liaise proactively and productively with external teams such as Coroners Service, Register Office, Council Bereavement Services and the Police.
  • Support the volunteers working within the service to enable them to feel valued members of the team.
  • Ensure a safe, clean working office/family rooms as well as accurate appointment booking, diary management and meeting bookings.
  • Place timely orders to meet the needs of the service such as office supplies or tickets to resolve IT or maintenance issues.

Person Specification

Qualifications, Knowledge, and Experience

Essential criteria

  • Good standard of secondary education with literacy and numeracy skills demonstrable to GCSE pass level or equivalent standard.
  • NVQ Level 3 in Business Admin or equivalent level of experience.
  • Experience of communicating with members of the public in highly emotional situations.
  • Proficient in all aspects of Microsoft Office.
  • Experience as a Receptionist or Admin Assistant linked to a clinical service or customer focused environment.
  • Knowledge of medical terminology.
  • Administrative and organisational experience.

Desirable criteria

  • Experience of working in a healthcare setting.
  • Experience of working as a member of a multi-disciplinary team.
  • Diploma level qualification or equivalent experience with evidence of additional personal development.
  • Proficient in the use of hospital computerised patient systems.
