Job Overview
We are looking for a motivated individual with excellent communication, interpersonal, and organisational skills, who is passionate about supporting patient care and working collaboratively as part of a busy and dynamic team.
Cancer MDT Trackers play a vital role in ensuring patients are progressed through their cancer pathway smoothly and within national waiting time standards. You will be responsible for monitoring and updating patient pathways from initial referral through diagnosis to treatment, working closely with Multi-Disciplinary Team (MDT) Coordinators, clinicians, and the wider Cancer Services team.
At any given time, the Trust is responsible for tracking hundreds of cancer patient pathways. This requires strong attention to detail, excellent time management, and effective communication to coordinate diagnostic tests, procedures, and admissions both within the Trust and with tertiary centres.

You will use a variety of IT systems to maintain accurate patient records, identify potential delays, and ensure all information is updated in real-time. Supporting the team to meet national cancer targets, you will contribute to the delivery of safe, efficient, and patient centered care.
Main duties of the job
- Accurately locate patient information from a range of systems to support the recording of all events along a patient's journey, highlighting any delays and escalating using agreed process.
- Proactively track and monitor patients along their cancer diagnostic and treatment pathways, ensuring timely progression in line with national Cancer Waiting Time standards.
- Accurately update and maintain patient records across a range of hospital systems, identifying and escalating any delays or pathway issues.
- Liaise with clinical teams, departments, and other Trusts to obtain information, confirm diagnostic plans, and help expedite appointments or investigations where required.
- Support MDT Coordinators with preparation for weekly Multi-Disciplinary Team meetings by identifying patients requiring MDT discussion.
- Review and action correspondence received via the generic NHS.net mailbox ensuring all cancer patients are correctly tracked on the database.
- Maintain the highest standards of data quality and confidentiality at all times.
- Participate in team meetings and contribute to service improvement initiatives.
Detailed Job Description And Main Responsibilities
The post holder will be responsible for managing their own workload, prioritising tasks effectively, and ensuring deadlines are met. They will be expected to work independently as well as part of a team, demonstrating flexibility and a commitment to high standards. The role may involve working across multiple sites and requires excellent communication and organisational skills.

Using the Trusts Cancer Management System, the postholder will track and monitor patients progressing them through their cancer pathway effectively and in line with both local and CWT guidance policies escalating any issues causing delays to patient pathways effectively and in a timely manner to seek resolution to the problem.
It is essential that all records are up to date and accurate which involves using multiple Trust systems to identify any information relevant to the patient and their cancer pathway working collaboratively with Op's, Clinical and Supporting Teams to obtain information and clarify plans for diagnostics and seek to expedite where appropriate all investigations, appointments to progress patients through to discharge or diagnosis.
Daily use of generic Cancer Team email boxes to update all relevant cancer PTL information received and to communicate requests for updates and escalations to relevant Teams both internal and external.
Adhere to processes that identify patients with a new diagnosis of cancer needing discussion at MDT to decide on treatment plans and management and highlight these to MDT Coordinators with full up to date information required for these discussions.
To ensure all diagnostics, appointments procedures requested are undertaken and interpret outcomes from these and effectively request clinical review for next steps.
