Specialist Adult Congenital Heart Disease (ACHD) Nurse
An exciting opportunity has arisen for a motivated and enthusiastic nurse to join our Cardiology Service as a Specialist Adult Congenital Heart Disease (ACHD) Nurse. Based across Eastbourne District General Hospital and Conquest Hospital, you will provide specialist care and support to adults living with congenital heart disease, working as part of a dedicated multidisciplinary team.
About the Role
This established specialist role offers the chance to make a real difference to patients’ lives while delivering high-quality, holistic and patient-centred care. We are seeking an experienced cardiac nurse with excellent clinical assessment, communication and organisational skills, alongside the vision and commitment to further develop ACHD services within the Trust.
Responsibilities
- Work across outpatient clinics and other clinical settings, supporting the delivery and ongoing development of specialist ACHD care.
- Manage a caseload of adults with congenital heart disease, providing specialist assessment, care coordination and ongoing support throughout their patient journey.
- Assess cardiovascular risk and patients with valve disorders; whilst working collaboratively with the physiology team within the valve clinic.
- Empower patients to understand and manage their condition through education and support, including providing advice on congenital heart disease, cardiac risk factor modification, healthy lifestyle choices and long-term health management.
- Assess psychosocial needs and provide appropriate support and signposting.
- Work closely with Consultant Cardiologists and a wide range of multidisciplinary professionals, including specialist teams at tertiary ACHD centres, to ensure the delivery of seamless, high-quality patient-centred care.
