Cardiac Specialist Nurse - Heart Failure Service

We are recruiting for a Cardiac Specialist Nurse - Heart Failure Service who shares our vision to be trusted to provide consistently outstanding care and exemplary service to our patients.

An exciting opportunity has arisen to join a team of dynamic cardiac specialist nurses. You will have experience in cardiology with an interest in heart failure to ensure patient-centred clinical care. This will encompass the skills of assessment, examination, diagnosis and treatment within an agreed scope of practice within Cardiology, focusing mainly on the assessment and treatment of inpatients and in the heart failure clinics to ensure optimisation of medication and arrange appropriate cardiology tests and follow up.

Key Responsibilities:

  • Support a new way of working that emphasises a more efficient and patient-focused service.
  • Ensure the safe referral and discharge of patients.
  • Provide expert Cardiology Nursing care, responsible for assessing, developing and implementing specialist nursing care programmes and undertaking specialist nursing procedures.
  • Provide specialist advice and support to, and liaise with, other staff, including the provision of specialist education and training for other staff and students.
  • Implement a clearly defined integrated approach to patient care encompassing new treatments and evidence-based care.
  • Undertake research and lead clinical audits in own specialist area.
  • Provide appropriate verbal and written patient information, thus empowering the patient to make informed decisions.
  • Promote the service both within the Trust and externally to the organisation.
  • Represent the trust and specialty at a regional and national level.
  • Provide cross cover within the specialist nursing team.
  • Work at different sites across East & North Herts NHS Trust, moving towards a 7-day service.
