Role Eligibility
Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.
About the Role
Join a dynamic Urgent Care Service that is transforming community healthcare. We provide short-term health, therapy and care support to help people remain safely at home, preventing unnecessary hospital admissions and supporting timely discharges.
Available 24/7, 365 days a year, our teams deliver compassionate, person-centred care to vulnerable patients, helping them regain independence and achieve the best possible outcomes in their own homes.
This is an exciting time to join us as we expand innovative services, including Virtual Wards, enabling patients with conditions such as COPD and respiratory infections to receive specialist care and remote monitoring at home.

Key Responsibilities
As a member of the Urgent Care team, you will play a vital role in supporting patients to remain safe, well and independent within their own homes and community settings. Working as part of a multidisciplinary team, you will deliver high-quality, person-centred care, ensuring care is provided in line with agreed care plans, risk assessments and professional standards.

- Provide practical and compassionate support, including personal care, meal preparation and medication administration
- Carry out clinical observations and delegated procedures
- Recognise changes in a patient’s condition, escalating concerns appropriately and providing timely feedback to senior clinicians
- Support the triage of referrals, undertake basic and follow-up assessments, develop care plans and risk assessments, arrange equipment and make onward referrals where required
- Gather patient feedback to support service improvement and maintain a strong understanding of Trust policies and procedures to promote patient safety and quality care

About the Organisation
Kent Community Health NHS Foundation Trust is due to integrate with Medway Community Healthcare on 1 October 2026. Following integration and to align our community services to the new Neighbourhood Model of care delivery, there will be a period of consultation which may result in changes affecting some services, teams or roles.
