Role
Care Coordinator – Caversham - £15.15p/h
This role is for 37.5 hours per week. The successful candidate will also feature on the on-call rota.

Responsibilities
As a Care Coordinator with us you'll play a pivotal role in ensuring the people we support are kept safe by ensuring our care delivery colleagues are deployed effectively and that all administration is undertaken in line with relevant regulations and legislation.
- Managing and coordinating of the care delivery team
- Ensuring that the relevant documentation is in place for any new people we support, discharges and transfers
- Conducting live call monitoring using bespoke software
- Being the first point of contact for day-to-day operational requirements of our care colleagues
- Working alongside the management team to coordinate with families, healthcare professionals, and external agencies
- Handling enquiries, scheduling appointments and meetings
- Handling all information with confidentiality

Requirements
This is an amazing role for someone who is looking to really make a difference to the lives of others. We’re interested in our Care Coordinators having both the right experience and the right values. This is why at this service our Care Coordinators should have:
- Experience working in Health & Social Care
- Experience in the use of Microsoft Office and be computer literate
- Prior experience in an administration role
- Experience providing excellent customer service
