Job Opportunity: Integrated Community Mental Health Teams (CMHTs) in Bradford

We are seeking autonomous and innovative practitioners to join our Integrated Community Mental Health Teams (CMHTs) in Bradford. Our service is committed to delivering recovery-focused interventions, supporting individuals on their personal recovery journey through creative and flexible approaches.

About the Role

You will work as part of a multidisciplinary team including Doctors, Nurses, Psychologists, Social Workers, Occupational Therapists, and Support and Peer Workers. Together, the team delivers care aligned to our Care Closer to Home model, ensuring accessible and person-centred support within the community.

The successful candidate will work directly with service users with severe and enduring mental health needs, acting as a Care Co-ordinator and playing a key role in supporting recovery. You will demonstrate excellent communication skills, a proactive approach to collaborative working, and enthusiasm for continuous service improvement.

Responsibilities

  • Delivery of Community Mental Health Care: Responsible for the delivery of community mental health care to a caseload of clients in accordance with the objectives of the Care Trust and within team operational policies.
  • Trust Policies and Procedures: Ensure implementation of all Trust Policies and Procedures as required.
  • Referral Management: To receive and accept referrals for the service and initiate, lead and co-ordinate their assessment, making referrals to other professionals as required.
  • Caseload Management: To manage a personal caseload of clients.
  • Care Coordination: To work as a Care Co-Ordinator responsible for assessment, planning, intervention, and evaluation of care for a designated client group. Undertake accurate, timely, and skilled assessments and ongoing reassessment of client’s condition, taking into consideration all social, physical, and environmental factors.
