Role Description
The Care Co-ordinator is responsible for organizing and overseeing care packages to ensure that each person receives timely, appropriate, and person-centered support. Day-to-day tasks include scheduling care visits, matching support workers to individuals’ needs, updating care plans, and maintaining accurate records in line with company procedures and regulatory standards. The role involves regular communication with individuals, families, care staff, and external professionals to coordinate services, respond to changing needs, and resolve queries or concerns promptly. The Care Co-ordinator will monitor service quality, support compliance with policies and safeguarding requirements, and contribute to continuous improvement of care delivery. This is a temporary, on-site role based in South Lanarkshire.

Qualifications
- Experience in care coordination, social care, or health and social care settings, with a strong understanding of person-centered and preventative care.
- Proficiency in scheduling, rota management, and maintaining care records using digital or paper-based systems.
- Strong communication and interpersonal skills for working with individuals, families, staff, and partner agencies.
- Ability to prioritize tasks, manage time effectively, and respond calmly to urgent or changing care needs.
- Knowledge of safeguarding, confidentiality, and relevant health and social care regulations and standards.
- Commitment to compassionate, dignified care and respect for the rights, choices, and diversity of the people supported.
- IT literacy, including basic use of email, word processing, and care management systems.
- Relevant qualification in social care, health and social care, or a related field (or equivalent experience); SSSC registration or eligibility is an advantage.
- Experience working within community-based or home care services in Scotland, particularly South Lanarkshire, is beneficial.
- Ability to work collaboratively as part of a multidisciplinary team and to contribute to service improvement initiatives.

Requirements
- Previous experience in homecare care coordination or a similar role is preferred.
- Basic office skills including proficiency in Microsoft Office applications.
- Must have a valid driver's licence and access to a car.
- Experience with rota or scheduling responsibility in a care setting is preferred.
- Experience supervising care staff is preferred.
- Experience with support plans and risk assessments is preferred.
- Familiarity with Care Inspectorate standards, self-directed support, and Scottish Social Service Council requirements is preferred.
- Eligible to work in the United Kingdom.
- Available to participate in a shared out-of-hours rota.
