Ready for a care coordination role where your experience can make a real difference?
Bluebird Care Medway & Swale is looking for an experienced Care Coordinator to join our office team. This is an exciting opportunity to take ownership of care rotas, solve the challenges that come with a busy service and help ensure customers receive safe, reliable care from people who know them.
You’ll work closely with Care Assistants, customers, families and colleagues across the office. We’re looking for someone who enjoys bringing order to a changing day, communicates clearly and understands that good coordination is about more than filling visits.
What you’ll do
- Plan and maintain care rotas at least four weeks in advance, matching Care Assistants with customers based on needs, preferences, availability and location.
- Allow appropriate travel time between visits and work to meet agreed visit times.
- Respond to daily changes, arranging cover for sickness, holidays and emergencies.
- Coordinate new care packages and adapt rotas as customers’ needs change.
- Keep customers, families and Care Assistants informed, particularly when plans change.
- Maintain accurate electronic records and document important conversations and decisions.
- Respond to coordination concerns and work with the wider team to resolve them promptly.
- Take part in the shared out-of-hours on-call rota in addition to your usual working hours.

What you’ll bring
- Experience in care coordination, along with a good understanding of what it takes to deliver a safe, person-centred service.
- Confidence using electronic systems, comfortable making decisions under pressure and able to balance the needs of customers and Care Assistants.
- Strong organisation and communication skills.
- Knowledge of care regulations and compliance requirements.
- A full UK driving licence with access to a vehicle.

What we offer
- £28,000 to £31,500 per year, depending on experience.
- Full-time hours, Monday to Friday, 8:30am to 5:00pm.
