Role Description

The role involves:
- Regular communication with clients, families, and healthcare professionals to ensure care is delivered safely, effectively, and in line with individual preferences
- Managing case records
- Monitoring service quality
- Responding to client queries and concerns
- Helping resolve issues promptly
This position also requires:
- Collaborating with the wider team to improve processes
- Maintaining compliance with relevant regulations
- Supporting positive outcomes for clients
Qualifications
- Candidates should possess skills in Care Coordination and Case Management to plan, organize, and monitor care services effectively.
- Candidates should possess strong Communication and Customer Service skills to interact professionally with clients, families, and colleagues.
- Candidates should possess knowledge of Home Care practices to support safe, person-centered services in client homes.
- Relevant experience in social care, healthcare, or community support settings is beneficial.
- A relevant qualification in health and social care (or equivalent experience) is preferred, along with familiarity with care regulations and safeguarding.
- Ability to work collaboratively, manage time effectively, maintain accurate records, and use basic computer systems is required.
