Job Title: Care Coordinator

Reports to: Care Manager

Salary: £26,000 - £30,000

Role Summary

To organise, coordinate and supervise care services in accordance with current best practice, company policies, legislation requirements and agreed standards. Provide leadership to care staff, maintain high-quality service delivery and participate in the on-call rota.

Entry Requirements and Qualifications

  • Applicants should typically be at least 18 years old with experience in a care environment.
  • Experience in a fast-paced work environment
  • Ability to be flexible
  • RQF Diploma Level 3 in Health & Social Care, or working towards it.
  • Background checks including DBS/CRB checks may be required.
  • Commitment to ongoing training and professional development.

Key Responsibilities

  • Ensure service users are at the heart of care delivery and that their wishes and preferences enhance their wellbeing.
  • Supervise staff and contribute to the efficient running of the service.
  • Organise and coordinate care delivery according to policies, procedures, regulations and contract requirements.
  • Ensure care is delivered at agreed times and for the specified duration.
  • Promote and protect the welfare of individuals supported by the service.
  • Provide leadership and support to care staff.
  • Maintain current professional skills and undertake required training.
  • Organise, coordinate and supervise care services within legal and contractual standards.
  • Participate in and maintain an on-call facility.
