Care Coordinator

About the Role
As a Care Coordinator, you will be responsible for coordinating care services, managing staff rotas, maintaining effective communication between clients and care staff, and ensuring the smooth day-to-day running of our care services.
Key Responsibilities
- Coordinate and manage staff rotas to ensure continuity of care and appropriate staffing levels.
- Schedule care visits and arrange cover for sickness, absences, and last-minute changes.
- Communicate effectively with clients, families, care staff, and management.
- Monitor care calls, staff attendance, punctuality, and scheduling issues.
- Maintain accurate records, care schedules, and administrative documentation.
- Respond promptly and professionally to queries, concerns, and changes in clients’ needs.
- Support care plan reviews and promote person-centred care.
- Assist with resolving staffing issues and escalating concerns where necessary.
- Maintain compliance with safeguarding procedures, company policies, confidentiality requirements, and relevant CQC standards.
- Support the smooth daily operation of domiciliary care services.
- Carry out spot checks and supervision of staff.
- Liaise with professionals, attend meetings with care professionals.
- Maintain accurate records of on-call activities, incidents, decisions, and actions taken.
- Participate in the on-call rota to provide support outside normal office hours, including evenings, weekends, and public holidays as required.

Essential Requirements
- A minimum of one year’s relevant experience in care coordination, domiciliary care, rota planning, or a similar health and social care role.
- Excellent organisational, time-management, and problem-solving skills.
- Strong written and verbal communication skills.
- Good IT skills, including Microsoft Office, email, and spreadsheets.
- A professional, empathetic, and client-focused approach.
- Ability to work independently and as part of a team.
- Must be a driver and hold a valid driving license.
- Understanding of confidentiality, safeguarding, and person-centred care.

Desirable Qualifications and Experience
- GCSE English and Maths or equivalent.
- Level 3 Diploma in Adult Care or an equivalent recognised qualification.
- Level 3 or Level 5 qualification in Leadership and Management for Adult Care.
- Previous experience using care management software or electronic rota systems.
- Knowledge of CQC regulations and domiciliary care procedures.
