Role Overview
We are looking for a friendly, organised, and motivated Care Coordinator to join our team on a full-time basis on-site and help keep our domiciliary care daily operation services running smoothly. You will play a key role in supporting our carers, reassuring our service users, and contributing to the team effort to make sure everything behind the scenes works as it should.

What You Will Be Doing
- Welcoming new service users with introductory calls, explaining:
- Who we are and how we support them
- The type of care they will receive (Reablement, ongoing homecare, TH&S, NHS)
- What to expect from their carers and how to contact us
- Setting up care tasks on our systems, especially medication tasks, working closely with carers and the Medication Champion.
- Answering calls from service users and carers, offering clear and supportive guidance.
- Acting as a PASS System Champion, helping the team use systems accurately and confidently.
- Running reports on spot checks and reviews.
- Monitoring internal messaging systems for reporting and follow up.
- Reporting and following up on repairs-related issues.
- Supporting carers during No Entry situations, following SOPs and keeping managers informed.
- Completing the daily tasks duty tasks and providing cover for colleagues when required.
- Taking part in out-of-hours phone support when needed.
Requirements:
- Excellent organisational and time-management skills.
- Strong communication and interpersonal skills.
- Ability to prioritise and multitask effectively.
- Attention to detail and ability to maintain accurate records.
- Ability to work flexibly and adapt to changing demands.
- Problem-solving skills.
- Previous experience in the sector is not required; full training will be given.
