Care Coordinator Brighouse, West Yorkshire

Up to £28,000 + Bonus

Full-time, Permanent Office-based Monday–Friday, plus on-call duties on rotation

The Role

You will schedule and coordinate care visits across our Complex Yorkshire Hub, making sure our clients get the right care at the right time. You will work closely with our Service Manager and Complex Case Managers, manage staff allocation, track attendance at supervision and training, and handle the on-call rota on a weekly basis.

What You Will Do Day-to-Day

  • Coordinate care visits using our scheduling system
  • Respond to staffing changes and new client requests quickly
  • Track staff supervision, training, and CQC-required timescales
  • Maintain contact with clients and ensure care plans are being delivered
  • Record concerns, complaints, and compliments
  • Cover on-call duties on rotation

What We Are Looking For

You Will Need

  • Strong organisational and time management skills
  • Ability to work with healthcare workers and manage competing priorities
  • Understanding of CQC regulations
  • Clear communication — written and verbal
  • NVQ Level 3 in Health and Social Care (or willingness to work towards it)
  • Willingness to participate in on-call duties
