Role Description
The Care Coordinator is a full-time, on-site role based in Gloucester, responsible for organizing and overseeing care services for clients receiving in-home support. Day-to-day tasks include assessing client needs, developing and updating care plans, scheduling Care Professionals, and ensuring continuity and quality of care. The Care Coordinator will communicate regularly with clients, families, and care teams to address concerns, adjust services, and maintain clear records of care activities. This role also involves collaborating with external providers as needed, supporting compliance with company policies and care standards, and contributing to ongoing service improvements.

- Demonstrated ability in care coordination and case management, including assessing needs and creating individualized care plans.
- Experience in home care or related health and social care settings, with understanding of in-home support for older adults.
- Strong communication and customer service skills, with the ability to build trusting relationships with clients, families, and team members.
- Organizational and time-management skills, including scheduling, prioritizing tasks, and maintaining accurate documentation.
- Proficiency with basic computer systems and scheduling or care management software.
- Ability to work collaboratively in an on-site environment in Gloucester and respond effectively to changing care needs.
- Relevant qualifications or training in health, social care, or a related field; prior experience in coordination or administrative roles in care is beneficial.
- Commitment to person-centered care, respect for diversity, and adherence to ethical and regulatory standards.
