Care Coordinator – Dually Diagnosed Program
Location: 150 Glenwood LN, Birmingham, AL 35242 Position Type: Full-Time
Glenwood is a private nonprofit organization providing a continuum of care for children and adults with autism spectrum disorders (ASD) and children with behavioral health disorders. We are seeking a dedicated Care Coordinator to support individuals in our Dually Diagnosed Program, which serves those with intellectual and developmental disabilities (IDD) alongside mental health conditions (MI).
Essential Functions
- Comply with the Code of Ethics, demonstrating respect and dignity for all clients, families, visitors
- Provide therapy/case management services, including:
- Individual, group, and family therapy
- Crisis intervention
- Attending face-to-face assessments, ISP meetings, support team meetings, and court hearings as needed
- Coordinate home visits and individual needs with legal guardians
- Maintain clinical records per agency standards and protocols
- Develop and coordinate treatment plans for assigned cases
- Accept and process referrals, conduct preadmission intake, and plan discharges
- Participate in psychiatric-led treatment team meetings
- Ensure timely completion of all administrative and clinical paperwork, including UMUR clinical reviews
- Adhere to professional development requirements and ethical guidelines
- Collaborate with colleagues, covering cases as needed, providing co-therapy, and offering clinical training to staff/interns
- Attend monthly mandatory house/program and divisional meetings
- Share on-call responsibilities with qualified mental health professionals
- Provide direct care, staff support, and supervision when required

Skills & Abilities
- Strong clinical skills for individual, group, and family therapy assessment
- Excellent interpersonal and communication skills
- Ability to work effectively with a diverse population
- Relates well with referral sources
- Proficiency in administrative tasks and computer systems
- Strong organisational skills, attention to detail, and problem-solving ability
- Skills in teamwork, time management, and professional collaborative behaviour
Education & Licensure
- Master’s degree in social work, or a related field
- Licensed Professional Clinical Counselor (ALC, LPC, LGSW, or LCSW)
Experience
- 3+ years of preferred clinical/mental health experience, with licensure or eligibility
