Experienced Care Coordinator

Location: Office based, Amersham Salary: Up to £29,000 per annum based on experience

The Role

As Care Coordinator, you will have a direct, positive impact on the lives of others. You won’t just be stuck behind a desk—you will be based both in the office and in the field, helping ensure the best possible outcomes for our clients by:

  • Guiding and mentoring care teams
  • Reviewing care plans
  • Gathering feedback from families

Key Responsibilities:

  • Supporting Care Teams:
    • Guide, mentor, and oversee care team members via supervisions, spot checks, and training
  • Client Engagement:
    • Conduct visits to assess care quality
    • Gather feedback, update care plans, and conduct risk assessments
  • Communication and Coordination:
    • Maintain clear communication within the team and with clients
  • Scheduling and Operations:
    • Coordinate rotas, manage leave, and ensure carers have fair schedules to minimise travel
  • Compliance and Quality Assurance:
    • Maintain accurate records and assist with on-call duties when required

About You

Successful Care Coordinators at Ashridge Home Care demonstrate:

  • A methodical problem-solving mindset
  • Exceptional organisational skills
  • A genuine passion for connecting and empowering people

Essential Experience & Skills:

  • Proven experience in care coordination, preferably in domiciliary care
  • At least 2 years’ experience in care, with strong knowledge of quality care practices
  • Experience with Dementia/Alzheimer’s care is crucial
  • NVQ Level 3 in Health & Social Care (preferred or in progress)
  • Proficient communicator, confident with IT systems (MS Office, care software) and familiar with CQC standards
  • An approach that prioritises empathy, proactivity, problem-solving, and adaptability
  • Valid driving licence with access to a personal vehicle (essential for visiting clients)

Additional Requirements:

  • Must possess the right to work in the UK (no Visa sponsorship available)
  • All shortlisted candidates will undergo an enhanced DBS check.
