The Care Coordinator is a full-time, on-site role based in Somerset. This role is responsible for organizing and overseeing care plans, coordinating services for individuals with learning disabilities and complex needs, and ensuring that support is delivered in line with regulatory and organizational standards. Day-to-day tasks include conducting assessments, maintaining accurate records, liaising with families and professionals, scheduling home care visits, and monitoring progress to ensure that each person’s needs are met. The Care Coordinator will collaborate closely with clinicians and support staff, respond to inquiries and concerns, and contribute to continuous improvement in the quality of care and client experience.
