Role Overview
We are seeking a dedicated and compassionate Care Coordinator to join our team. The successful candidate will play a vital role in organizing and managing patient care plans, ensuring seamless communication between clients, healthcare professionals, and support services.
Key Requirements
- Coordinate and monitor individual care plans in collaboration with healthcare providers and clients
- Schedule appointments, follow-ups, and support services to meet client needs
- Maintain accurate and up-to-date records of client interactions and care activities
- Provide occasional hands-on care when necessary.
- Act as a point of contact for clients, families, and healthcare professionals to address queries and concerns
- Organize documentation, reports, and correspondence related to client care
- Assist with administrative tasks such as data entry, filing, and managing schedules using office software
- Ensure compliance with organizational policies and confidentiality standards
- Support the on-boarding of new clients by providing necessary information and guidance
