Are you an organised, compassionate and proactive healthcare professional looking for a rewarding role where you can make a real difference to patients and their families?
We are looking for an experienced Care Co-ordinator to join a specialist palliative care service supporting patients and families across the local community.
This is an important role within a 24-hour advice and support service, helping to ensure patients receive timely, compassionate and well-coordinated care. You will work closely with registered nurses and a wide range of healthcare and community professionals to coordinate referrals, support care pathways and help patients remain in their preferred place of care wherever possible.
You will also play an important role in supporting joined-up working and helping to prevent avoidable hospital admissions through effective communication and coordination between services.
About the role
As a Care Co-ordinator, you will:
- Coordinate referrals and support patient care pathways.
- Triage and prioritise referrals alongside a registered nurse, considering urgency, complexity and risk.
- Act as a key point of contact for patients, families, carers and healthcare professionals.
- Respond to clinical enquiries by telephone and provide appropriate information and signposting.
- Ensure patients receive a welcome call and relevant service information.
- Monitor and manage referrals and email communications.
- Support the smooth flow of information between healthcare services and professionals.
- Liaise with GPs, community teams, ambulance services, hospices, clinical specialists, social care and voluntary sector organisations.
- Recognise signs of deterioration, red flags and safeguarding concerns, escalating appropriately.
- Promote coordinated, person-centred care for patients and their families.
- Maintain accurate records and report accidents and incidents appropriately.
- Work within your scope of practice while maintaining confidentiality and professional standards.
- Contribute to the delivery of a high-quality palliative and end-of-life care service.

About you
You will have experience of coordinating appointments, referrals or patient pathways, ideally within a healthcare or community setting. You will understand the principles of palliative care and the importance of providing holistic, person-centred support to patients and families.
We are looking for someone who has:
- A good standard of literacy and numeracy, with GCSE English and Maths or equivalent.
- A Level 3 qualification in Health & Social Care.
- Experience working within multidisciplinary teams.
- Experience handling confidential information.
- An understanding of palliative care and primary healthcare services.
- Knowledge of confidentiality and information governance.
- Excellent communication and interpersonal skills.
- A professional and friendly telephone manner.
- The ability to work both independently and as part of a team.
- The ability to remain calm and effective under pressure.
- An accurate, methodical and organised approach to your work.
- The ability to prioritise competing demands and adapt to changing situations.
- Good problem-solving skills and IT skills.
- An understanding of adult safeguarding.
- A willingness to take direction, undertake delegated duties and escalate concerns appropriately.
