Role Description

The Care Coordinator role is a full-time, on-site position based in Eridge Green, responsible for organizing and overseeing care services for clients within the local community. Day-to-day tasks include scheduling care visits, matching caregivers to clients based on needs and preferences, and maintaining accurate care plans and records. The Care Coordinator will liaise with clients, families, and caregivers to ensure care plans are followed and adjusted as circumstances change. The role involves managing inquiries, resolving issues promptly, and providing high-quality customer service to maintain trust and satisfaction. The Care Coordinator collaborates closely with the management team to support compliance, continuity of care, and efficient service delivery.

  • Strong care coordination and case management skills, with experience organizing and monitoring individual care plans.
  • Preferably NVQ3
  • Knowledge of home care services and an understanding of best practices in person-centered care.
  • Excellent communication skills, including clear verbal and written communication with clients, families, and colleagues.
  • Customer service skills, with the ability to handle enquiries, resolve concerns, and maintain positive relationships.
  • Ability to work on-site in Eridge Green, managing multiple tasks, priorities, and schedules in a fast-paced environment.
  • Attention to detail and strong organizational skills, including accurate record-keeping and use of care management systems.
  • Previous experience in a care coordination, home care, or similar role is a plus.
  • NVQ 3 in social care for adults is a plus too.
  • Own car.
