Care Coordinator
📍 Location: E14 9TS
💰 Salary: From £26,000 per year
📅 Job Type: Full-time, Permanent
About the Role
As a Care Coordinator, you'll play a key role in ensuring our customers receive safe, high-quality care in their own homes. You'll coordinate care schedules, support our care teams, and work closely with customers, families, and healthcare professionals.
Key Responsibilities
- Coordinate care assistant rotas and schedules
- Allocate carers to customers based on skills, availability, and customer needs
- Manage new care referrals and changes to care packages
- Arrange cover for sickness, holidays, and emergencies
- Monitor care delivery to ensure visits are completed on time
- Maintain accurate records using IT systems
- Liaise with customers, families, and healthcare professionals
- Support the on-call rota (following training)
- Assist with general office administration and reporting

What We're Looking For
- Previous experience as a Care Coordinator in social care
- Excellent organisational and communication skills
- Good IT and administration skills
- Ability to work under pressure and solve problems
- Caring, compassionate, and customer-focused approach
- Knowledge of confidentiality and safeguarding
- Full UK driving licence and business insurance
- Reliable, professional, and flexible

Desirable
- Qualification in Health & Social Care and/or Management
- Knowledge of safeguarding procedures
