Job Introduction
Are you highly organised, a confident communicator and someone who enjoys helping people?
We have a full-time opportunity for a Care Coordinator to join our Care Co-ordination Centre at Gravesham Community Hospital.
You’ll be an important first point of contact for service users, GPs and Integrated Health and Social Care teams across North Kent, receiving and processing referrals and making sure information reaches the right services within the required timeframes.
Working in a busy environment, you’ll communicate with a wide range of people throughout your day, answering queries, accurately recording information and supporting the smooth running of our community services.
This is an ideal opportunity for someone with strong administrative and customer service experience who enjoys working as part of a team and wants their work to have a positive impact on patients and healthcare colleagues.
The Care Co-ordination Centre operates 7 days a week between 8am and 8pm. You’ll need to work flexibly across these hours, including weekends and Bank Holidays, as well as Christmas, New Year and Easter holidays as part of the rota.

As a Care Coordinator, you’ll be part of our valued team based at Gravesham Community Hospital, playing an important role in helping patients and healthcare professionals access the right community services.

Main Responsibilities
As a Care Coordinator, you will:
- Act as a first point of contact for service users, GPs and Integrated Health and Social Care teams
- Receive and process referrals accurately and within required timeframes
- Ensure referrals and information are directed to the appropriate services and teams
- Respond professionally and helpfully to telephone and other enquiries
- Communicate effectively with patients, healthcare professionals and colleagues
- Maintain accurate and up-to-date information on relevant systems
- Provide administrative support to help ensure the efficient day-to-day running of the service
- Manage competing priorities within a busy Care Co-ordination Centre
- Handle sensitive and confidential information appropriately
- Work collaboratively with colleagues to provide a responsive and efficient service
- Provide a high standard of customer service at every point of contact
- Identify queries or concerns that require further support and escalate them appropriately
- Work flexibly across the service's operating hours, including weekends and Bank Holidays

The Ideal Candidate
You’ll be a friendly, organised and dependable individual who can remain calm and professional while working in a busy environment.
You’ll enjoy communicating with people and understand how important a positive first experience can be for patients, families and healthcare professionals contacting our services.
You will have:
- Previous administrative experience within a busy, customer-facing environment
- Excellent verbal and written communication skills
- Excellent customer service skills
- A professional and confident telephone manner
- Strong organisational skills with the ability to manage competing priorities
- Good attention to detail and the ability to accurately record information
- The ability to work effectively as part of a team
- A flexible and adaptable approach to your work
- Confidence using IT systems
- A working knowledge of Microsoft Office packages, including Outlook, Word and Excel
- The ability to handle confidential and sensitive information professionally
Most importantly, you’ll be committed to providing an excellent service and helping patients and healthcare professionals get the support they need as efficiently as possible.
You’ll need to be able to work flexibly across our 7-day service between 8am and 8pm, including weekends and Bank Holidays as part of the rota.
