Role Description
The Care Manager is a full-time, on-site role based in Merton, responsible for overseeing high-quality care services delivered to clients in their own homes. Day-to-day tasks include:
- Managing client care plans
- Conducting assessments and reviews
- Coordinating visits
- Ensuring that care is personalized, safe, and aligned with regulatory standards

The role involves:

- Supervising and supporting Care and Support Workers
- Providing guidance, coaching, and performance feedback
- Organizing and delivering training as needed
The Care Manager will:
- Liaise with clients, families, and external professionals
- Respond promptly to concerns or incidents
- Maintain accurate documentation and reports
This position requires active participation in service improvement initiatives and contribution to a positive, collaborative team environment.
Qualifications
- Demonstrated skills in case management, including developing, implementing, and reviewing individualized care plans.
- Experience in training and supporting care staff, with the ability to deliver induction and ongoing development sessions.
- Knowledge of elder care and social services, with an understanding of community-based support and safeguarding principles.
- Basic understanding of medicine relevant to home care, including medication administration protocols and coordination with healthcare professionals.
- Previous experience in a care management or supervisory role within home care or social care settings is highly beneficial.
- Strong communication, interpersonal, and conflict-resolution skills, with the ability to build trust with clients, families, and colleagues.
- Excellent organizational skills, attention to detail, and ability to manage multiple cases and priorities effectively.
- Relevant qualification in health and social care (e.g., NVQ/QCF Level 3 or above) or equivalent experience; willingness to pursue further accredited training.

- Ability to travel locally within Merton as required to visit clients and staff, in line with role responsibilities.
