Role Description
The Care Manager is a full-time, on-site role based in Nottingham, responsible for overseeing the delivery of high-quality, individualized care services. Day-to-day tasks include assessing client needs, creating and updating care plans, coordinating care schedules, and monitoring service quality to ensure safe and effective care. The role involves close collaboration with clients, families, and multidisciplinary professionals, providing guidance, support, and regular communication about care progress. The Care Manager will lead and support care teams, including providing supervision, coaching, and training to ensure best practices are consistently followed. Additional responsibilities include maintaining accurate documentation, ensuring regulatory and company compliance, and contributing to service improvement initiatives.

- Demonstrated skills in case management, including assessment, care planning, and coordination of services.
- Experience in training and supporting care professionals, with the ability to deliver guidance, coaching, and ongoing development.
- Knowledge and practical experience in elder care, with a strong understanding of age-related needs and person-centered support.
- Background in social services, with familiarity in community resources, safeguarding principles, and support frameworks.
- Foundational understanding of medicine related to home care, including basic health monitoring and working with clinical professionals.
- Strong communication, organization, and problem-solving skills, with the ability to manage multiple cases and priorities.
- Previous experience in care management, home care, or related health and social care settings is highly beneficial.
- Relevant qualification in health and social care, social work, nursing, or a related field is preferred, along with willingness to pursue ongoing professional development.
