About The Role

With the upcoming Support at Home reforms, now is the time to make a real impact on the lives of older Australians. If you're passionate about enhancing quality of life and shaping a strong clinical service, this role is for you.

As a key member of our team, you'll coordinate, monitor, and support the clinical care needs of clients receiving home care services. You'll also manage a caseload of Home Care Package clients and work closely with Care Partners to assess and address clinical needs.


Some Of Your Responsibilities Will Be

  • Conducting regular reassessments to ensure that clients’ needs are being met effectively
  • In collaboration with the Scheduling team, coordinate the delivery of services such as personal care, meal preparation, cleaning, and transportation
  • Monitoring clients’ funding, taking corrective action to ensure services are provided within funding and supporting the management of unspent funds
  • Adhering to Aged Care standards and guidelines, and organisational policies and procedures
  • Conducting regular team meetings, performance reviews, and ongoing training to ensure high standard of care delivery

About You

To be a successful candidate you will have:

  • Registered Nurse, Enrolled Nurse, Allied Health and Social Work, minimum of 3+ year's experience in aged care – Home or residential aged care
  • Ability to showcase comprehensive knowledge and practical experience in client care, clinical procedures and healthcare protocol
  • Proficiency in an additional language, particularly Arabic, is desirable
  • Experience in residential care and dementia care is desirable
  • Experience in home care, case management, or a related clinical setting. At least 2 years of supervisory or leadership experience
  • Strong leadership, communication, and interpersonal skills and ability to work collaboratively in a multidisciplinary team setting
