Care Team Leader

Location: To cover areas within the Torfaen Borough

About The Role

As a care team leader, you’ll play an important part in supporting our customers, care professionals and office team.

This is a full-time, field-based role, with most of your working day spent out in the community. You’ll get to know the customers and care professionals in your area, building positive relationships and being a regular point of contact when they need support.

From carrying out customer reviews and helping to introduce new care packages to supporting care professionals with their wellbeing and development, you’ll help us provide safe, reliable care that reflects what matters to each customer.

You’ll work closely with the office team too, responding when needs change and helping with the day-to-day running of the service.

Supporting our customers

You’ll:

  • Carry out customer reviews within your allocated area.
  • Get to know customers and their families and build positive relationships with them.
  • Recognise and report changes in a customer’s needs.
  • Support the introduction of new care packages.
  • Help establish new care runs when needed.
  • Work closely with the office team so customers receive safe, reliable and consistent care.

Supporting our care professionals

You’ll:

  • Welcome and support new care professionals during their shadowing period.
  • Give new team members practical guidance and encouragement as they build their confidence.
  • Be a regular point of contact for care professionals in your area.
  • Spend around 10 hours each week supporting care professional wellbeing through regular welfare conversations.
  • Share concerns about wellbeing or performance with the care manager or registered manager.

Supporting the wider service

You’ll:

  • Work closely with the office team when customer or staffing needs change.
  • Complete records accurately and on time.
  • Raise concerns about customer wellbeing or the delivery of care.
  • Help the team provide high-quality care across your allocated area.

Working Pattern And On-call Responsibilities

As part of this full-time role, you’ll take part in our on-call rota and will need to be able to work alternate weekends as part of your usual working pattern.

For On-call Duties, You’ll Receive:

  • £25 on-call allowance for each weekday.
  • £50 on-call allowance for each weekend day.
  • A nine-week rotating weekend on-call rota, giving you regular weekends away from on-call duties.

During your allocated on-call periods, you’ll respond to emergency calls and provide advice and support when needed.

What You’ll Need

For this role, you’ll need:

  • A full UK driving licence and access to a car.
  • To be aged 18 or over.
  • To be able to work alternate weekends.
  • To be able to work our shift pattern.

Pay

You’ll Receive:

  • £14.25 per hour for contact time.
  • £12.71 per hour for non-contact time.
  • £16.50 per hour on bank holidays.
  • 35p mileage allowance.
