Age Connects Torfaen is a charity that supports older people living in Torfaen and surrounding boroughs, focusing on helping individuals get the support they want, when they need it. Its services are guided by what older people themselves identify as most important, ensuring that activities and advice are relevant and person-centered. The organization offers opportunities to engage through volunteering on frontline services or contributing to governance as a Trustee, depending on how people wish to be involved. A friendly and knowledgeable Information & Advice team is available Monday to Friday, providing guidance on a wide range of issues affecting older people. Role Description
The Carers Officer is a full-time, on-site role based in Torfaen & Blaenau Gwent, supporting unpaid carers of people living with dementia by providing innovative short-breaks. The Carers Officer will provide a coordinated, accessible, and responsive short-break service for unpaid carers across Torfaen and Blaenau Gwent.

The postholder will act as a Single Point of Access for carers seeking short breaks, working closely with carers to understand their individual circumstances, identify their needs, and arrange appropriate short-break opportunities.

The role will ensure that carers are supported to access meaningful breaks that can help maintain their wellbeing, reduce stress and isolation, and support them to continue in their caring role.
The Carers Officer will work closely with carers, families, volunteers, community organizations, statutory services, and other partners to ensure that short-break provision is person-centred, flexible, and responsive to individual needs.
Funded through Carers Trust Wales Shortbreaks Scheme, the role is fixed term until the 31st March 2029.
Qualifications
- Experience supporting carers or working with older people in health, social care, charity, or community settings.
- Strong communication and interpersonal skills, including the ability to listen, build trust, and engage sensitively with people in challenging situations.
- Ability to carry out needs assessments, maintain accurate records, and produce clear written reports.
- Good organizational and time-management skills, with the capacity to manage a caseload and prioritize competing demands.
- Knowledge of local health, social care, and voluntary sector services, or willingness to develop this understanding quickly.
- Competence in using standard office software and databases, and confidence in handling telephone and in-person enquiries.
- Commitment to equality, diversity, safeguarding, and person-centered practice.
- Relevant qualification in social care, health, community work, or a related field, or equivalent experience.
- Ability to work both independently and as part of a multidisciplinary team, and to contribute positively to service development.
- Willingness to travel locally as required and to work within the policies and procedures of Age Connects Torfaen.
