We are recruiting for a dynamic and enthusiastic Case Co-Ordinator to join our mentoring team.

This role plays an integral part in the effective running of case co-ordination and behaviour management plans for the young people supported in our hub here at Kingsholm.

  • Follow guidance from the Head of Mentoring, Mentoring Manager & DSL
  • Ensure that weekly/termly reports are completed to an acceptable standard and shared with commissioning parties in a timely manner.
  • Developing and updating behaviour management plans in collaboration with the mentoring manager and the DSL.
  • Be responsible for initial individual profiles and the updating of these.
  • Have responsibility to ensure that individual case files have all required documentation such as EHCP’s, referrals, Risk Assessments.
  • Ensure session update chats are undertaken and outcomes are relevant in these communications.
  • Undertake initial visits and meetings if required.
  • Collaborate with Pastoral Lead to ensure that consistent strategies are used with learners.
