Complex Care Nurse Case Manager – Brighouse / Yorkshire Region
£45,000 per annum + bonus | Full-time | Permanent
A specialist healthcare recruitment agency is currently working in partnership with a leading complex care provider to recruit an experienced Registered Nurse Case Manager. This is an excellent opportunity for a clinically confident nurse to step into a field-based leadership role supporting high-acuity patients across the community setting.
The Role
You will be responsible for overseeing and coordinating complex care packages in the community, ensuring safe, person-centred, and high-quality care delivery. Working across Yorkshire, you’ll act as the clinical link between service users, care teams, and external professionals.
This role offers a strong blend of hands-on clinical oversight, care coordination, and leadership within a supportive and structured environment.

Key responsibilities
- Managing and reviewing complex care packages in the community
- Developing, updating, and implementing detailed care plans and clinical risk assessments
- Supporting care teams through clinical guidance, training, and competency assessments
- Attending multidisciplinary meetings and liaising with healthcare professionals and commissioners
- Conducting audits of care documentation, medication records, and compliance standards
About you
- Registered Nurse with an active NMC PIN
- Experience in high-dependency, acute, or critical care environments
- Tracheostomy and ventilation experience (community experience advantageous)
- Strong organisational and communication skills
- Confident IT and documentation abilities
- Working knowledge of CQC standards
- Full UK driving licence and access to a vehicle
