RTR is offering a $2,500 Sign on Bonus for New Full Time Hires!

Support and evaluate adults with developmental disabilities in a day habilitation setting. Member of an interdisciplinary team in the coordination of therapeutic services.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:

  • Review each recipient’s treatment and habilitation goals monthly and inform staff of any changes in the recipient’s status.
  • Supervise the implementation of the ISP/DHSP and maintain the individual’s records.
  • Coordinate OT, PT, Speech, Nursing and behavioral services for the assigned caseload of individual members.
  • Case Management responsibilities include reviewing or writing monthly progress notes, writing two year Day Habilitation Service Plans, writing assessments and objectives, and completing semi-annual reviews.
  • Assist with all development of behavioral plans or other intervention strategies.
  • Chart data collected from programs.
  • Maintain contact with DDS service coordinators and families of individual members.
  • Prepare reports in accordance with state and agency requirements.
  • Attend mandated Safety Care training.
  • Responsible for implementing physical restraints on the individuals we support to assist with behavioral management. This may include, but not limited to, hitting, biting, kicking, and spitting.
  • Work in cooperation with Direct Care Staff to develop member skills in the areas of: self-help skills, sensorimotor skills, communication skills, independent living skills, affective development skills, social development skills, behavioral development, and wellness.
  • Serve as a liaison between RTR and the community representing a positive and pro-active attitude at all times.
  • Serve as an appropriate role model for individuals under supervision establishing behavioral limits in a positive consistent manner and in accordance with behavioral plans.
  • Participate in the Positive Behavior Support initiative and complete the Quality Behavior Curriculum.
  • Provide transportation to community activities as needed.
  • Attend and participate in all meetings concerning assigned individuals.
  • Adhere to all program routines and schedules.
  • Assist and support individuals as needed with physical direct care/dietary needs, may include: physical or mechanical transfer of individuals from wheelchairs to a van, chair, toilet etc. May also include any nursing activity that is deemed to be delegable, including, but not limited to, the administration of thickening agents, and assistance with special diets.
  • Implement all behavioral plan interventions as assigned, these behavioral interventions may include physical restraint.
  • Cooperate and interact with co-workers, clients, volunteers, and funders in a collaborative, courteous, empathic, and professional manner. Demonstrate willingness and ability to be part of team effort.
  • Adhere to the schedules and needs of RTR and the schedules and needs of individuals under supervision.
  • Adhere to agency safety standards.
  • Perform additional duties, or substitute for others as required. Additional duties may include but are not limited to clearing snow and ice from walkways, vehicles, parking lots, and roads using equipment like shovels and snow blowers while applying deicing agents to ensure safety.
  • Adhere to all RTR policies and procedures.
  • Develop and provide all services in accordance with RTR’s philosophies including staff and Board beliefs.

Qualifications

QUALIFICATIONS

  • Must have experience with case managing and case reviews in a relevant health care setting.
  • Valid Driver’s License.
  • Basic knowledge of Microsoft Word and the internet.
  • Successful completion of all state and agency mandated trainings.
  • Must be willing to authorize a Criminal History Background Check (CORI) and release of information concerning allegations from Disabled Persons Protection Commission (DPPC), Department of Developmental Services (DDS), and other agencies.
  • Must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform these essential functions.
  • Tuberculosis screening within the previous 12 months.

LANGUAGE SKILLS

Must be fluent in English and have the ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write advanced reports and correspondence. Ability to communicate effectively with individuals under supervision, employees of the organization, families, and members of the community.
