Contract: Permanent, full time
What are we looking for?
Cleveland Clinic London is seeking a Case Manager to join our Case Management team. This role offers a competitive salary range based on experience and is full-time, Monday to Friday, 9am – 5pm, onsite.
Responsibilities
- Coordinate patient care and discharge planning from admission through to discharge for key patient groups.
- Act as a liaison between patients, families, clinicians, and third-party payers.
- Manage a complex patient caseload, conducting utilization reviews and pre-authorizations.
- Optimize length of stay and reimbursement outcomes.
- Work collaboratively with multidisciplinary teams to address barriers to care and discharge.
- Proactively identify risks to patient flow and coordinate complex transitions of care.
- Support the achievement of clinical, operational, and financial performance objectives.

What will your duties include?
- Assess, plan, coordinate, and review patient care and discharge planning from admission through to discharge.
- Act as the key liaison between patients, families, clinicians, and third-party payers.
- Manage a complex caseload, identifying and addressing barriers to discharge.
- Develop effective working relationships with external stakeholders.
- Contribute to service improvement initiatives, innovation, research activities, and audit programs.

What we need from you?
- Bachelor's Degree in Nursing.
- Current registration with the Nursing & Midwifery Council (First Level) with substantial post-registration experience.
- Senior-level clinical experience with demonstrable leadership, service coordination, and/or team supervision responsibilities.
- Considerable post-registration inpatient bedside nursing experience.
- Proven ability to lead complex clinical decision-making, work autonomously, and influence multidisciplinary teams.
