Case Manager | Nursing & Care

Location: Central London
Salary: Up to £42,000 per annum

Compass Associates are delighted to be working with a quality-focused nurse-led private nursing and care provider who are looking to appoint an experienced, permanent Case Manager to manage a portfolio of private clients across Central London.
This is a fantastic opportunity for an experienced care professional to take on a critical role with clients who expect consistently excellent communication, responsiveness and service delivery. The business is prioritising finding the right person to maintain its high standards and continue to improve the service.

The Role

As Case Manager, you will manage a portfolio of private clients who require visiting care alongside some live-in care with a small number of complex cases. Working as part of a large team you will collaborate to ensure high service and care levels to uphold the companies credibility in this space.

Responsibilities

  • Managing an allocated portfolio of private clients and prospective clients, covering both visiting and live-in care, completing client assessments, risk assessments and personalised care plans.
  • Explaining services, processes and costs clearly, from initial enquiry through to service delivery.
  • Reviewing care delivery regularly and responding to changing client needs.
  • Managing your own diary, balancing an existing caseload with new enquiries.
  • Undertaking quality assurance visits, audits and care plan reviews.
  • Managing complaints, incidents and service quality concerns, implementing timely corrective action whilst maintaining the reputation of the business and high standards of service.
  • Promoting safeguarding, dignity, independence and regulatory compliance.
  • Communicating confidently with clients, families and healthcare professionals.

Requirements

  • NVQ Level 3 in Care as a minimum qualification.
  • Experience delivering and supervising care services, including live-in care.
  • Strong experience managing care packages and complex cases.
  • Knowledge of person-centred care planning, risk assessment, safeguarding and regulatory compliance.
  • Excellent communication and relationship-building skills.
  • Able to manage complaints and resolve issues while maintaining positive client relationships.
  • Strong organisational and digital skills, including experience using care management systems.
  • Sound professional judgement when assessing complex or changing care needs.
  • Private-sector or high-expectation client experience would be advantageous; NHS or local authority experience considered where relevant.
  • Enhanced DBS clearance and the right to work in the UK (sponsorship cannot be provided).
