About the role
Support children and young people in Out-of-Home Care through case management, therapeutic planning, and frontline coaching. Drive positive outcomes by embedding trauma-informed, culturally safe practices and ensuring compliance with standards and legislation.
The role operates across two primary functions:
- 18 hours per week supporting frontline staff in the homes to implement case and care strategies.
- 20 hours per week completing case work duties, including planning, documentation, and stakeholder engagement.

🫶 What You’ll Be Doing:
• Lead and review case plans to meet therapeutic and cultural goals • Coach and support staff to embed strategies in daily care • Engage with families, carers, and agencies to align goals • Maintain documentation and ensure compliance with standards • Promote safe, nurturing, and culturally respectful environments
💼 What You’ll Bring:
• Diploma (or working towards) in community, youth, or social work • Proven case management and stakeholder engagement skills • Knowledge of trauma-informed and therapeutic care practices • Strong documentation and compliance capability • Current full Australian driver’s licence and probity clearances
