About The Service
Our Stepdown Service in Headington provides short-term supported accommodation for people following discharge from hospital. The service helps individuals who are medically ready to leave hospital but require support to secure suitable accommodation and continue their recovery in the community.
The impact you will have
As a Case Worker, you will support people to transition safely from hospital into more independent living. Working in a person-centred way, you will help individuals access housing, benefits, health and wellbeing services, activities, and wider support networks that enable them to build confidence and maintain their recovery.
Key responsibilities include:
- Supporting clients to identify goals and develop plans for independent living.
- Helping people access accommodation, benefits, healthcare, and community resources.
- Providing practical and emotional support tailored to individual needs.
- Working collaboratively with hospital teams, local authorities, housing providers, and other partner agencies.
- Supporting clients to build confidence, independence, and positive support networks.

Contract details
- Contract: Permanent
- Hours: 37.5 hours per week
- Salary: £27,418- £28,541, per annum, dependent on experience.
- Location: Headington, Oxford
- Closing Date: Thursday 10th September
- Introduction calls: Monday 14th September
- Interviews: Friday 18th and Monday 21st September

About You
We're looking for someone who is compassionate, patient, and committed to helping people move forward in their lives. You'll be a good listener who can build positive relationships, work in a person-centred way, and support individuals to achieve greater independence following discharge from hospital.
You'll bring:
- Experience of working within housing, homelessness, mental health, social care, substance misuse services, or supporting people with offending histories.
- An understanding of housing applications, welfare benefits, and tenancy responsibilities.
- The ability to build positive relationships and support people to achieve their goals.
- Good organisational skills and the ability to maintain accurate records and support plans.
- Basic IT skills, including the use of Outlook and Word.
- An understanding of the causes of homelessness and a commitment to delivering person-centred support.

A full UK driving licence and access to a vehicle are essential, as the role includes supporting people to attend appointments and access services. Your main base will be our service in Headington.
