Caseload Coordinator - Urgent Community Response & Virtual Ward: Permanent (30 hours)
This position is for 1 part time 30 hours per week post, some weekend working is required.
This is an exciting time to join our team, supporting us to care for patients safely in their own home avoiding unnecessary admissions and facilitating early discharges from hospital.
UCR/HaH is a multi-disciplinary team that provides a two-hour urgent response to patients in their own home that are experiencing a crisis. The service can clinically monitor patients remotely via Virtual Ward using enhanced technology.
This role requires a highly organised person, with ability to prioritise their workload and work in a calm and effective manner and communicate well at all levels.
Important Notice on Visa Sponsorship Eligibility
Please note that whilst the trust does offer sponsorship this role is unfortunately not eligible for visa sponsorship under the UK Health and Care Worker/Skilled visa route. This is due to the position not meeting the minimum salary threshold of £25,000 per annum and/or the required skill level set by UK Visas and Immigration.

This role will involve managing the case load of clinical staff within the Urgent Community Response (UCR) and Hospital at Home (HaH) Team. It will involve gathering, inputting and providing essential data for patients, helping to maintain accurate records and enhancing the team’s ability to provide personal care.
The role is responsible for the administration support and caseload co-ordination within the team to support the delivery of care to patients. The post holder would also be required to answer the telephone, which may include calls from patients, and information then passed onto a clinician.
