Role:

To provide an efficient and effective Case Management, Care and Interventions Programmes Service that meets with Childrens Homes Regulations, Children’s Homes National Minimum Standards, Youth Justice Standards, Leeds City Council and the homes policies, procedures and guidance. Promote and reinforce effective practice ensuring that the Work Force Development Strategy is being met and appropriate training is accessed and developed.

Responsibilities

• Contribute proactively to the positive development of children and young people through high quality assessment, planning and provision of Care and Interventions Programmes.
• Assess individual needs and provide appropriate interventions to meet the level of the complex issues and risks associated with the behaviours identified.
• Develop and maintain specialist input and services that are in the best interests of the child or young person and be able to communicate these effectively to all stakeholders.
• Monitor and evaluate interventions designed to promote, positive options, desistance from offending and Effective Practice
• Develop and sustain effective working and information sharing protocols with colleagues and external partner agencies to ensure an integrated service is provided, delivered and maintained.
• Monitor and evaluate engagement skills and the competency of practitioners and students on placement through observation and feedback.
• Monitor and evaluate the engagement of young people and families through assessment and intervention planning and delivery.
• Develop self to enhance performance and practice through reflection and learning, share knowledge, best practice, promote ideas for innovation and improvement and use information to take effective decisions.
• Support the Casework and Interventions Manager in coordinating and monitoring the scheduling of reviews, ensuring they are held within timescales.
• Assist the Casework and Interventions Manager in the maintenance of a database detailing each child and young person’s Care and Interventions Programme, recording progress.
• Manage difficult situations and people who display volatile and challenging behaviour
• Communicate, engage and build positive relationships with staff, children, young people, their families and other professionals.
• Participate in developing and maintaining resource materials for Care Planning and Interventions and Staff Development.
• Deliver 1:1 and group interventions, ensuring consistency of delivery to allow for effective practice and evaluations.
• Work with colleagues to evaluate and analyse the transitions process.
• QCF Assessor (willing to work towards).
• To physically restrain and control in line with approved restraint techniques, guidelines and procedure (training will be provided).
• To participate in annual appraisal, training and development activities as necessary to ensure up to date knowledge and skills.
• Ensure prior to using any equipment or undertaking an activity a risk assessment is in place and a safety data sheet available if required.
• In line with the Medication Policy and Procedure administer and record prescribed and other medication if required.
• Contribute to the development of the Casework and Interventions Service
• Provide reports, attend and chair meetings and reviews as required.
• Deliver Casework and Interventions services by the use of current software programmes and packages.
• To undertake any other duties that, are commensurate with the job evaluation outcome for this post.
