Job Title: Certified Nursing Assistant (CNA) (Full Time, Evenings)

Department: Short Term Rehab

Location: Clifton Springs Nursing Home, 2 Coulter Rd, Clifton Springs, NY 14432

Hours Per Week: 37.5

Schedule: Evenings, 3p-11p, M-F, Every other weekend

Signing Bonus: Up to $4,500

SUMMARY

As a Certified Nursing Assistant, you will provide high quality, individualized patient care in collaboration with the nursing team. Your ability to take initiative while always assisting and maintaining patient care will be critical in the success of the entire team.

RESPONSIBILITIES

  • Reads and follows the resident’s care card for each of assigned residents for daily personal care needs, treatments and therapies. Follows up with the nurse for clarification of information and direction. Offers suggestions if sees a better way to meet resident’s needs.
  • Gives personal care to residents: bathing, toileting, feeding, transferring, and ambulating. Promote resident’s involvement and independence to the extent the resident is able.
  • Assists residents with personal services: personal laundry, keeping room neat, operating television, radio, reading and/or writing letters.
  • May assist relatives and visitors, directing questions regarding the patient’s clinical condition to the supervising nurse.
  • Encourages residents with any self-help measures in all activities of daily living.
  • Assists with and/or performs procedures as outlined and according to policy with proper instruction and supervision such as: admitting, transferring, discharging residents; taking vital signs, collecting specimens such as urine and stool, changing colostomy bags, changing non-sterile dressings, bagging soiled briefs, measuring and recording intake and output, caring for residents on precautions giving care to dying residents and deceased residents.
  • Uses standard precautions.
  • Encourages resident to talk about their interests and concerns and to reminisce about subjects that provide resident with satisfaction.
  • Participates in interdisciplinary care conference when able.
  • Observes and reports to nurse resident’s physical condition, behavior, reactions and appetite and any changes from usual. Reports any hazardous condition that may jeopardize resident, visitor, or staff safety.
  • Transports residents throughout the facility as needed.
  • Attends in-services and is fulfills the necessary number of hours of state required in-service.
  • Is cooperative, courteous and assists all members of the team.
  • Floats throughout the facility when necessary.
  • Performs other duties as assigned.

PREFERRED QUALIFICATIONS:

  • Strong customer service and communication skills highly desired.
  • High School diploma or equivalent.

MINIMUM QUALIFICATIONS:

  • Minimum age requirement: 18

  • Currently certified in New York State as a Certified Nursing Assistant

PHYSICAL REQUIREMENTS:

  • Moderate - Clinical: Moderate clinical roles are patient facing roles that require continual movement, including standing, walking, and lifting 20–50 lbs., along with pushing and pulling medical equipment and assisting with patient positioning. Employees perform frequent bending, twisting, and reaching while completing fine motor tasks in busy or confined clinical environments. High sensory acuity is essential for monitoring patient status and responding to alarms or safety concerns. Emotional and cognitive demands are substantial, involving decision-making under pressure, responding to emergencies, and maintaining sustained focus. Environmental exposures include infectious agents, bodily fluids, chemicals, and radiation. TB testing, mask-fit testing, and physical exams are common requirements.
