Role Description

Manage a caseload of participants in custody providing high quality support and guidance, enabling participants to engage and break down barriers to successful resettlement, achieve agreed milestones and develop enhanced skillsets, leading them into an ultimate goal of sustainable employment.

What You Will Be Doing

  • Responsible for managing a caseload of participants with complex needs, ensuring the delivery and coordination of all achievements including induction, assessment, support work, training and education, pre-release preparation and employment sourcing/support.
  • Updating of case notes including risk and inputting of achievements onto CATS+ the commissioners case management recording system, ensuring a high level of compliance to avoid rejected achievements.
  • Ensuring that P-Nomis, the prison case management recording system, is updated where relevant with work carried out with the participant, and any information relevant to the participant, such as achievements, actions, safety and security.
  • Building, managing and maintaining local stakeholder relationships within the prisons and local community, to ensure a steady flow of referrals/support and ensure that there is no duplication of services.
  • Build, manage and maintain relationships with other Creating Future Opportunities (CFO) Evolution staff (both externally and internally) to ensure smooth transition for prison through the date, including arrange pre-release 3-way meetings and supporting with Release on Temporary Licence (RoTL).
  • Support with the identification, resourcing, and coordinator of partner delivery for CFO Evolution participants.

What We Need From You

Must have/be able to:

  • Level 2 English & Maths or equivalent experience
  • Demonstrates a strong understanding of the barriers faced by prisoners and prison leavers with complex needs, providing effective support, motivation, and guidance to help individuals progress towards education, training, employment, and successful reintegration, while working collaboratively with partner agencies
  • Delivers high-quality services in line with contractual requirements, using excellent communication, organisational, IT, and problem-solving skills to engage stakeholders, achieve targets, manage risks, and drive positive outcomes.
  • Demonstrates flexibility, resilience, and initiative in challenging environments, with a commitment to continuous improvement, safeguarding, Equality, Diversity and Inclusion, and maintaining high professional standards.
