Job Overview
The Band 6 Children's Hospital at Home (HaH) Nurse will provide high-quality, child-centred nursing care to children and young people within their own homes and community settings as an alternative to hospital admission or to facilitate early supported discharge.
The post holder will work autonomously and collaboratively within a multidisciplinary team to assess, plan, implement and evaluate care for children with acute and chronic health conditions. They will act as a clinical leader, supporting junior staff and students, promoting evidence-based practice, and ensuring the delivery of safe, effective, and compassionate care.
The role requires excellent clinical assessment skills, sound decision-making abilities, and the capacity to work independently while maintaining strong links with acute paediatric services, primary care, and community partners.
Main Duties of the Job
- Deliver specialist nursing care to children and young people in their homes, schools, and community settings.
- Undertake holistic assessments, formulate care plans, and evaluate outcomes in partnership with children, young people, and families.
- Provide acute assessment and monitoring of children referred to the Hospital at Home service.
- Safely administer medications, including intravenous therapies where appropriately trained.
- Support the management of acute childhood illnesses in accordance with agreed pathways and clinical guidelines.
- Identify signs of clinical deterioration and escalate concerns appropriately.
- Facilitate early supported discharge from inpatient wards and the Children's Emergency Department.
- Support admission avoidance initiatives through enhanced community-based care.
- Maintain accurate and contemporaneous clinical records in accordance with Trust policies and professional standards.
- Participate in safeguarding children processes and contribute to child protection assessments where required.
