Job Overview

Are you an experienced registered healthcare professional looking to use your clinical expertise to make a meaningful difference to individuals with complex health and care needs?

We have an exciting opportunity for a CHC Clinical Case Manager to join our AACCC service, supporting the delivery of NHS Continuing Healthcare (CHC), Funded Nursing Care, and complex individualised commissioning.

This is a varied and influential clinical role encompassing CHC assessments and reviews, Fast Track pathways, care planning and commissioning, complex case management, appeals, and retrospective reviews. You will provide specialist clinical expertise while helping ensure individuals receive high-quality, person-centred care that reflects their assessed needs.

Working alongside clinical colleagues, system partners, and the AACCC Senior Leadership Team, you will contribute to service improvement, performance, governance, and the effective implementation of the National Framework for NHS Continuing Healthcare.

We are looking for an experienced, confident, and compassionate clinician with strong knowledge of CHC and NHS Funded Nursing Care and evidence of clinical leadership.

Current professional registration with an appropriate regulatory body is essential, and the successful candidate will be required to undertake an Enhanced DBS check.

Main duties of the job

As a CHC Clinical Case Manager, you will provide specialist clinical expertise across NHS Continuing Healthcare, Funded Nursing Care, and complex individualised commissioning, supporting the effective implementation of the National Framework for NHS Continuing Healthcare.

You will work across a range of clinical workstreams including CHC Fast Track Pathway assessments, reviews, care planning, and clinical commissioning, complex case management, appeals, and retrospective reviews.

Working with the AACCC Senior Leadership Team, you will contribute to the leadership, performance, and development of the service, providing specialist advice and supporting improvements in quality, governance, and patient outcomes.

You will coordinate and contribute to internal and external working groups, manage clinical and operational risks, analyse complex information, and support the development of policies, procedures, and service improvement initiatives.

The role includes supporting performance and resource management, contributing to business and financial planning, and ensuring accurate and timely reporting.

You will also provide leadership to colleagues, supporting recruitment, induction, appraisal, training, and development, while promoting effective multidisciplinary and partnership working across health and care services.

You will work autonomously, applying professional clinical judgement to complex situations while maintaining confidentiality, professional standards, and a person-centred approach.

Detailed Job Description and Main Responsibilities

The post holder will be responsible for assisting the AACC Senior Leadership Team with leadership, implementation, and management of the National Framework for Continuing Healthcare and Funded Nursing Care.

The role will require travel on occasions; access to transport is therefore necessary, be it personal or public.

Qualifications

Essential criteria

  • Educated to degree level in relevant subject or equivalent level qualification or significant experience of working at a similar level in specialist area with appropriate regulatory body registration e.g. Nursing Midwifery Council for Registered Nurses, Registered Mental Health or dual Registered RN/RMN/RNLD, Allied Health Professional, Midwife, or Social Workers.

Experience

Essential criteria

  • Significant experience of successfully operating in a politically sensitive environment.
  • Demonstrated experience of coordinating projects in complex and challenging environments.
  • Evidence or experience of clinical leadership in a health or complex environment.
  • Experience and knowledge of developing and implementing policy including standard operating processes.

Desirable criteria

  • Evidence of continuing professional development.
  • Demonstrated experience of coordinating projects in complex and challenging environments.

Skills

Essential criteria

  • Ability to prepare and produce concise yet insightful communications for dissemination to senior stakeholders and a broad range of stakeholders as required.
  • Knowledge of financial systems e.g. monitoring budget management, processing invoices, and procurement.
  • Demonstrated capability to plan over short-, medium-, and long-term timeframes and adjust plans and resource requirements accordingly.
  • Demonstrated capabilities to manage own workload and make informed decisions in the absence of required information, working to tight and often changing timescales.
