Job Overview
The post will provide medical cover for rehabilitation patients in our community hospitals.
The post is primarily based within the community hospitals in Flintshire – Mold and Deeside community hospitals which provide a combination of general and stroke specific rehabilitation. The successful applicants will join a team of two Speciality Doctors, one ANP, and two trainee ANPs to provide cover for patients within our community hospitals.
Main duties of the job
The primary duties will be overseeing the health, ongoing rehabilitation, and discharge planning of the patients in the hospital.

Detailed Job Description And Main Responsibilities
The post will provide medical cover for rehabilitation patients in our community hospitals.
The post is primarily based within the community hospitals in Flintshire – Mold and Deeside community hospitals which provide a combination of general and stroke specific rehabilitation. The successful applicants will join a team of two Speciality Doctors, one ANP, and two trainee ANPs to provide cover for patients within our community hospitals.
Each hospital has between forty and forty-eight patients at any one time depending on the demand. There will be two clinicians at any one time covering each community hospital, and leave is co-ordinated to ensure this. Where there is a surplus of staff within the community sites it is expected that staff will work within Wrexham Maelor Hospital, either to support other services, or to gain experience in other areas.
Senior cover is provided by consultant physicians and consultant therapists who carry out ward rounds at least once a week. Patients in the community hospitals are medically well, and are undergoing rehabilitation or discharge planning. No Intravenous medications are given in the community hospitals and as such any patients that are unwell are transferred back to district general hospitals.

The primary duties will be overseeing the health, ongoing rehabilitation, and discharge planning of the patients in the hospital.
The post is full time, with no on-call commitment, although this can be negotiated at Wrexham Maelor Hospital if the applicant desires.
The appointee will be able to attend educational sessions within the main Geriatric Medicine department including a weekly radiology MDT and Geriatric Medicine teaching program.
The Post holder will have the opportunity to attend clinics within the Geriatric Medicine department if desired. These include a wide variety of sub-specialities such as: stroke and TIA, falls, movement disorders, and general geriatrics.
The post holder will be responsible to the consultant he/she is working for and through him/her to the clinical lead for care of the elderly.
